Info for consultation for Project/business: Business name  as it appears on Business licence and other names the business might use., business address full address (include city, country, apt, zip code), date business started legaly. All business partners full name and date of birth (DOB) as it appears on ID.
 
Info for consultation of pets: Name of pet, DOB, full address (include city, country, apt, zip code)
